The first week home

Seven days, planned before discharge

We plan the first week from the hospital's discharge letter, so the first day home is not the first day of working it out.

  1. Day 1

    Settling in

    A carer meets you at home, gets the bed and the bathroom ready and stays for two hours.

  2. Days 2 to 4

    Daily visits

    Washing, meals and company, at the times written in the care plan.

  3. Day 5

    A check with the family

    A short call to go over how the week is going.

  4. Day 7

    What next

    We agree with you whether to carry on, change or stop the visits.

Do your carers give medicines?

Why is there an assessment visit first?

So the plan fits the person and the home, and so the carer knows what to expect on the first visit.
