Before any care starts
The assessment visit
A care coordinator visits at home, listens to the person and the family and writes a care plan with you. Nothing is booked until you have read it.
The visit
What happens in the hour
A cup of tea first
The coordinator introduces themselves and explains what the visit is for. There is no rush.
A normal day
Waking, washing, meals, moving about the house, sleep, and what the person enjoys and does not.
The home
Steps, the bathroom, the bed and the chair, so we know where a grab rail or a stool would help.
The care plan
A written plan with visits, times and an example fee, sent the same day for you to read.
In the care plan
What you receive
One page, in plain words, that the carer reads before the first visit.
- Visits
- Which days, what time and how long
- Tasks
- What the carer helps with on each visit
- Preferences
- Food, faith, language and the name the person likes to be called
- Contacts
- Family, the person's doctor and who to call first
- Review date
- Two weeks after the first visit
- Fee
- Per visit and per week. Example fees for this demo store.
About the assessment
Is it a medical examination?
No. It is a conversation about daily life at home.
Who should be there?
The person who needs care, and anyone in the family who helps.
What if we decide not to go ahead?
